Dogmatic Opinion
Most of the time, the thing I imagine going POKKO-RI is a belly.
It has a childlike body-shape quality: the chest may be fairly flat, but the abdomen alone sticks out noticeably because of food, drink, or sometimes even malnutrition.
I do not think mere swelling above the surroundings is enough. There needs to be a clear contrast between the flatter area and the rounded bulge.
I would not normally describe breasts or nipples as POKKO-RI.
The protrusion is rounded, not sharp.
